What cagrilintide is

Amylin is a 37-amino-acid hormone co-secreted with insulin from pancreatic beta cells. Native human amylin is difficult to work with: it is strongly amyloidogenic, aggregating into fibrils in solution, which is why the earlier analogue pramlintide substituted three proline residues to break that behaviour.

Cagrilintide takes the same problem further. It is a 32-residue analogue carrying amino-acid substitutions that suppress aggregation, C-terminal amidation, and — the defining feature — a C20 fatty-diacid chain attached via a linker. That lipid chain binds reversibly to serum albumin, which is what converts a hormone with a half-life of minutes into one with weekly kinetics. The same albumin-binding strategy appears across modern peptide analogue design.

Its receptor pharmacology is worth stating precisely, because "amylin receptor" is a shorthand. Amylin signals through complexes of the calcitonin receptor paired with receptor activity-modifying proteins — AMY₁, AMY₂ and AMY₃, depending on which RAMP is present. Cagrilintide is a non-selective agonist across those complexes and at the calcitonin receptor itself, which is why it is described in the literature as a dual amylin and calcitonin receptor agonist rather than a pure amylin mimetic.

Cagri pen: the same thing under a shorter name

Most people searching for this product type “cagri” or “cagri pen” rather than the full compound name, and the two mean the same thing. Cagri is the common short form of cagrilintide, in the same way reta is the short form of retatrutide. A cagri pen is a pre-filled cagrilintide cartridge in a dial-dosing pen, as opposed to lyophilised powder in a vial.

This pen holds 10 mg of cagrilintide in a sealed 3 ml cartridge at 3.33 mg/ml. The dial is graduated at 15 clicks per 500 mcg, so the cartridge dispenses 20 increments of that size. Those are device figures — what the mechanism displaces, not what should be dispensed.

Cagrilintide is routinely compared with semaglutide, and the two are not variants of one another. They target unrelated receptors:

• Cagrilintide is an agonist at calcitonin-receptor-based amylin complexes. Amylin's role is in the regulation of gastric emptying and satiety signalling, acting substantially through the area postrema in the brainstem.
• GLP-1 analogues act at the GLP-1 receptor, driving glucose-dependent insulin secretion, glucagon suppression and slowed gastric emptying.

The reason the two appear together in the literature is that they are complementary rather than equivalent — separate pathways converging on the same physiology, which is precisely what makes a combination interesting to study.
