What cagrilintide is
Amylin is a 37-amino-acid hormone co-secreted with insulin from pancreatic beta cells. Native human amylin is difficult to work with: it is strongly amyloidogenic, aggregating into fibrils in solution, which is why the earlier analogue pramlintide substituted three proline residues to break that behaviour.
Cagrilintide takes the same problem further. It is a 32-residue analogue carrying amino-acid substitutions that suppress aggregation, C-terminal amidation, and — the defining feature — a C20 fatty-diacid chain attached via a linker. That lipid chain binds reversibly to serum albumin, which is what converts a hormone with a half-life of minutes into one with weekly kinetics. The same albumin-binding strategy appears across modern peptide analogue design.
Its receptor pharmacology is worth stating precisely, because "amylin receptor" is a shorthand. Amylin signals through complexes of the calcitonin receptor paired with receptor activity-modifying proteins — AMY1, AMY2 and AMY3, depending on which RAMP is present. Cagrilintide is a non-selective agonist across those complexes and at the calcitonin receptor itself, which is why it is described in the literature as a dual amylin and calcitonin receptor agonist rather than a pure amylin mimetic.
